I hereby grant permission to HiveMind Sport Solutions Inc., hereinafter referred to as the “Media,” to use my and/or my child’s image (photographs and/or video) for use in Media publications, including but not limited to:

- Social Media
- Website and/or Affiliates
- Email Blasts
- Recruiting Brochures
- Newsletters
- Magazines
- General Publications
I understand that the images may be used in both printed and electronic media.
I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image.
By submitting the form and payment for the service, I consent to the use of my and/or my child’s image as described in this Media Release Consent Form.
